Transitions of Care Policy

  • Hand-offs occur twice a day on the inpatient services. We use email for routine hand-off and direct conversation to discuss acute issues. This occurs between attending physician(s) and the pediatric cardiology fellows, as well as nurses.
  • For seriously ill patients, hand-offs have been conducted at the bedside between both the pediatric cardiology fellows and the attending physician(s).
  • First-year pediatric cardiology fellows are shadowed the first week of fellowship and oriented by a senior fellow regarding hand-offs  as well as by the directly supervising attending physician. The first year pediatric cardiology fellow also is oriented by the divisional administrator regarding the hand-off of nurse-triaged calls in EPIC.
  • The pediatric cardiology fellow and the attending call schedule are available in the UF Health Shands online on-call system, and is updated as indicated.
